Add constraint to prevent creation of duplicate global labels

      Currently, it is possible for duplicate global labels to be created in the database, typically from a malfunctioning plugin. This causes errors similar to this:

      2010-08-12 15:32:12,612 ERROR [TP-Processor28] [com.atlassian.hibernate.HibernateObjectDao] doInHibernate There is more than one label named 'test-label' in the 'global' namespace.

      A means to prevent this from happening would be beneficial.
