Automatically add a user to the reviewers list if they approve/decline a pull request and weren't already on the list.

Scenario:
Team member creates pull request and does not allocate a specific reviewer; leaving the wider team to pick up the review.
A team member then reviews the ticket and approves/merges.

Problem: Without looking at the specific detailed history of a pull request, you cannot see who approved the pull request.
