Separate email notification for each commit (BB-1914)

Having separate diff email for each commit would make searching through emails easier, especially for those using Gmail. Now it's difficult to see what's been changed without opening each email.

Moreover current behaviour is inconsistent - you get different title depending on how many changesets you push.
