Track reviewer unapprove events in PR activity feed

      Currently, if a reviewer approves a PR, then clicks "Unapprove",

      1. We are removing the initial approval event from the PR activity feed
      2. We're not tracking the unapprove event.

      It would be good to have these 2 events tracked separately in the PR activity feed for audit purposes.
