Feedback cannnot be submitted

Steps to Reproduce

1. Click on the Give feedback button on the right-hand navigation panel of the source page
2. Add feedback information
3. Click 'Submit'

Expected Results

• Feeback should be submitted

Actual Results

• Feedback is not submitted

The below message is thrown:

Oops! Something went wrong...

There was a problem submitting your feedback, likely due to the configuration of this form. You might want to contact the site owner to let them know about this issue.

Workaround
