Today I found a pull request which I was certain I had created but it said the author was someone else. After some enquires with reviewers and some testing I found that when you create a pull request with a source/target branch combination for which there's already a PR in the system, the original has its description and reviewers updated without warning.

Expected behaviour: the user is blocked from creating the pull-requests with a message indicating that one already exists.
