Features to help keep quarantined tests in check

As discussed on EAC, while quarantining tests is a great feature, there is potential for abuse that could be better controlled with a couple of enhancements:

1. Quarantining could (should?) require text explaining why the test is quarantined so other people will know and can help address it later. This should not live buried off in the original build; it should belong to the quarantine record so that it can show up in all builds where it is applied.
2. It should be possible to time bomb quarantines so that the test failures show up again once the quarantine period has expired.
3. It should be possible to limit the number of tests in the quarantine so that there is further motivation to keep them under control.
