Make repositories distinguishable by name in Slack integration form

Problem Definition

If your repositories have identical name it's impossible to select the proper repository in the Slack integration form.

Example:

The select will only show 3 entries named »Website«. This is also a flaw in the search of Bitbucket pull request form (at least you see the project name there).

Suggested Solution

Prefix the repository with the project name.
