DOCTYPE declaration included twice for issue related pages

    Description

      If you go to "View Issue" and view the source, there are two DOCTYPE declarations:

      <!DOCTYPE HTML PUBLIC "-//W3C//DTD HTML 4.01 Transitional//EN" "http://www.w3.org/TR/html4/loose.dtd">
















<!DOCTYPE HTML PUBLIC "-//W3C//DTD HTML 4.01 Transitional//EN" "http://www.w3.org/TR/html4/loose.dtd">

      This is due to issuesummary.jsp decorator including header.jsp decorator. Should simple remove it from issuesummary.jsp. (i think). Should check other decorators to ensure that only one declaration is included.
