It should be possible to run MS-SQL with case sensitive collations

      Because of the way MS-SQL server implements collations, when we run JIRA on MSSQL Server with a case sensitive collation then table name lookups start failing due to Ofbiz assuming case insensitive table names. This forces us to use Case Insensitive collations for SQL Server, which causes other interesting problems....
