Display transition name in change history

      Some users have multiple transitions between a single pair of statuses. That's why the change history tab should also include the transition name. For example:

      Change it from:

      Old Status New Status
      Open In Progress

      to:

      Old Status Transition New Status
      Open Start Progress In Progress
