Simultaneously editing the author, moderator and adding a reviewer results in BatchUpdateException: Duplicate entry

Steps to reproduce:
1. Edit a Crucible review
2. Change the author and moderator to person X
3. Add person Y as a reviewer
4. Click on Done
5. Author, moderator and reviewers remain unchanged and error log shows an exception

Expected behaviour:
Requested changes are made

Error log exception:
Caused by: java.sql.BatchUpdateException: Duplicate entry '1825-1' for key 2

Workaround:
Do the changes separately, ie. click on Done after step 2, then edit the review again and re-start at step 3.
