NPE when adding file to the review

Occasionally when we try to add files to a review, the file loading indicator spins forever and we see this in the log. This seems to happen to various files, no apparent pattern. Do you know what could cause this? I have seen it happen on different Fisheye repositories, only on large code reviews.
