Deferred upgrade tasks might not get executed if cluster nodes are upgraded concurrently

Documentation says that cluster nodes should be upgraded individually. If this advice is ignored and multiple nodes are started simultaneously, deferred upgrade tasks might not get executed on some nodes.

This situation should be detected and either serialised or the start-up should be stopped with an error.
