Links are not the right colour and underlining is inconsistent on QA-EAC

Link colours and underlining methods are inconsistent and there is a blue underline underneath the header (ala Fisheye). These affect both Firefox and IE8 on QA=EAC.

The impact of this bug is that the affordance of links now varies throughout the product - some are underlined, some aren't. This makes it more difficult to immediately see what is and isn't a link. Given how pervasive links are throughout the product, I would classify this as a pretty major usability problem. The colour is less of an issue but would presumably be fixed by the same thing.
