Add mobile sign up screen and link to it from mobile login screen

      Feedback from mhodges, when Confluence has public signup enabled, should give the signup link on the mobile login screen, eg: https://s3.amazonaws.com/uploads.hipchat.com/10804/74993/lczgmnlm38yklxb/confluence-signup.jpg

      Ideally we would have a responsive UI for the login page, so the same page functions as the login and signup pages for both desktop and mobile.
