Accessing an issue collector form directly through the URL using the custom template shows a blank page.

Steps to Reproduce

1. Create an issue collector using a Custom template.
2. add at least one Custom field to this issue collector (Description, Component, priority, attach file)
3. While editing the issue collector, get its ID in the URL.
4. Access the issue collector directly using the following URL:

   https://<INSTANCENAME>.atlassian.net/rest/collectors/1.0/template/form/<ISSUE COLLECTOR ID>
