Multiple Epic Label fields on Create Epic screen

As seen on QA-EACJ with the "Design" project, on QA-JAC with the "Angry Birds" project.

Steps to repro

1. Do not have Epics enabled (clean instance)
2. Have multiple screen schemes which reference the same screen for the Create operation
3. Have multiple issue type screen schemes that reference those multiple screen schemes for all issue types (or Epic issue types)
4. Associate those multiple issue type screen schemes to multiple projects
5. Enable Epics feature
