Release burndown: Incorrect counting of work added

Work added is sometimes being duplicated in the release burndown chart.

This occurs when:

• there are duplicate "added to version" events in an issue's change history (this can occur when multiple fix versions are being assigned to an issue).
• an issue is transitioned to "done", then "not done", before being added to a version
