Issue Summary

When a form (in a JSM request) containing an attachment field is left open for several minutes, attaching a new file fails and Send button becomes greyed out.

Steps to Reproduce

1. Create a form and include Attachment field
2. Add the form to a request type
3. Open the request type and leave it open for 20 minutes
4. Attach a file - Upload bar won't have any progress and Send button will be greyed out

Expected Results

Attachment is uploaded and user is able to send the request

Actual Results

Request creation is blocked by the Form Attachment

Workaround

Remove the Attachment field from the Form and add it to the Request type
