      Summary:

      I would like to have the flexibility to choose the fields that I want to include on the issue collector form. As an example, on the Got feedback template, I want to remove the attach file option. 

       

      Suggestion:

      Provide an option or additional page where the user can select the fields he wish to include on the issue collector form.
