When creating an issue the users randomly get a java.lang.reflect.UndeclaredThrowableException. It is difficult to reproduce and happens quite rarely and randomly. From the stacktrace it seems related to the data layer and problems setting the correct value for the primary key in OS_WFENTRY. The key value for this table looks OK in the sequence_value_item table (>max value from OS_WFENTRY table).
