Issue collector dialog height renders incorrectly sometimes

Summary

Steps to Reproduce

1. Create an Issue Collector with several custom fields
2. Open the issue collector on a live site

Expected Results

The issue collector will display a dialog that shows most or all of the fields on the form

Actual Results

The dialog is very small and only shows one field, and you have to scroll and can't see anything. It's awkward.

Notes

We have several documented cases where the JIRA Issue Collector does not display correctly. It's not tall enough for the number of fields that are included in the form, and produces some awkward scrolling.
