Default avatar is broken for instances that use Gravatar and are not publicly accessible

Sometime in September, Gravatar has made changes to how it handles the default avatar URL. The upshot is that some JIRA instances now have broken avatars for users that do not have a Gravatar.

This affects JIRA instances that are using Gravatars, and are not accesible on the public internet.
