Workflow stop working with InvalidActionException

Downloaded 5.1.2 tar.gz standalone for Linux.
Setup:

License generate an evaluation, did login, generated one and confirmed to install it.

Right away, did the following steps after that:
Administration
Issues->Workflows
entered password and confirmed
Create copy of jira Workflow : My Workflow
Workflow Schemes
Add Worflow Scheme: My Scheme
Associate an Issue Type with a Workflow
All unassigned ... to My Workflow
Projects -> Add Project
My Project, Key = MP
Workflows -> more -> Actions -> Select a Scheme -> My Scheme, did association
Exit Administration

Now:
Create Issue
Summary : First
Hit enter key

Issue MP-1 First has been successfully created

Clicked that link to go directly to that newly created issue.

Now did this:
Start Progress
Stop Progress
Resolve
Reopen
Start Progress
Stop Progress

ERROR

See attached error log and screenshot. Workflow error remains unless the
the issue gets reloaded. However, resolve, reopen, start and stop again
and same problem occurs again and again.
