Comment group/role level select appears in the wrong place on bulk operations screen!

On the bulk operations screen, the commentLevel select box renders at the top instead of inline with the comment field. This was broken in the comment-edit.vm template. The controlFooter macro for the entire block needs to come after the commentLevel select box for the second part of the template (the #else part).
