Revert locking change to SwitchableTokenManagerImpl

Partially revert the change in CWD-3522 that used the reader/writer lock to distinguish between reading and writing operations.

The lock was intended to distinguish between use of the underlying token store ("reading") and switching between implementations ('writing"). Return to that behaviour and improve the documentation in the class.
