The "Add all x revisions" option is broken

Steps to reproduce

1. Create a review with "Search for Files" option e.g go to Create review > Add content > Search For Files"
2. It happens when you have multiple revisions to the same file
3. Try to search using the advance query:

   select revisions from dir / where (in or before any open,closed review) and comment matches "MP-1" 
   order by date desc group by changeset return path, revision, author, date, csid
   

4. See the screen shot, the "Add all 4 revisions" doesn't work as expected
