Table of Content (TOC) macro's "Include" and "Exclude" option not used in Export

Description: The export of the ToC Macro ignores the include/exclude settings.

Steps to reproduce issue:

1. Create a page with a few headings, some of them with "-" and some without
2. Place a TOC macro on the page and set "Include Headings" to "-"
3. Place another TOC macro on the page and set "Exclude Headings" to "-"
4. Export the page.

Expected:

1. The first TOC macro displays only headings with "-" in it
2. The second TOC macro displays only headings with no "-" in it

Actual:

1. The first TOC macro displays no headings at all
2. The second TOC macro displays all headings
