[CONFSERVER-10619] toc macro doesn't work properly in preview mode

The toc macro seams to use the saved version of a page, so if you edit a page and preview it, the table of contents is not updated.

Create a page with a table of contents
Edit the page and add new toc elements
Preview the page, the toc is not updated
Save the page, the toc IS updated
