Image on Page breaks after renaming the file

      Issue Summary

      Images on a page break after renaming them and then editing and making changes to a page. This is intermittent and will not happen for all pages.

      Steps to Reproduce

      1. Create a page. Add an image in the page. Publish the page.
      2. Go to files-> click on `properties` of the attachment. Rename the attachment and hit save.
      3. Go back and edit the page and make some changes. Can also optionally publish the page again.

      Expected Results

      Image should keep displaying as before.

      Actual Results

      Image shows up as "Preview Unavailable"

      Workaround

      Delete the image from the editor and re-upload into the editor.
