When an image is removed in editor it will still be attached.
Suggestions: 

• • ask user if the attachment should also be removed when the last instance of the image on the page is removed*

Additional:

• show a flag in attachments which attachment is actually present on page
• shortcut under attachments to jump to a given the image on the page
• preview images in attachments without clicking on each file
