Symptoms
Context paths are missing, and connecting to Confluence via Apache's mod_jk fails. However, connecting to Confluence using http://localhost:<port> works.
Cause
Confluence 4.0's upgrade manager does not copy over any modifications from the previous installation's server.xml, including AJP connectors, context paths, etc.
Workaround
Ensure there is a backup of server.xml before upgrading to 4.0. After the upgrade, proceed to replace the server.xml in Confluence4InstallationDir/conf with the backup copy.

Thanks for taking the time to raise this issue. As you may be aware this has been on our backlog now for quite some time with very little progress being made, rather than leave this here I'm going to close this issue out as won't fix, I believe this better reflects the status of this issue.
If the problem is raised again in the future we'll readdress it.
