Add in Coverage Explorer: Checkbox to autoload per-test coverage.
Add in Test Run Explorer & Test Contributions: Banner if per-test coverage is out of date allowing coverage reload or allowing user to turn auto-load on. On user-prompted reload, full coverage is loaded (but not subsequently unless also prompted by the user).
