Migration/Setup: "A database error has occurred" masks useful failure details

Certain types of errors during database migration or setup are incorrectly handled, resulting in them being wrapped in a blanket "A database error has occurred" message which hides all the useful details. This also prevents the details from appearing in the logfiles, making such failures difficult to track down and fix.
