DefaultCustomFieldManager escapes this in constructor

DefaultCustomFieldManager registers itself with event publisher in its constructor, thus lets this reference escape before being fully constructed (see http://www.ibm.com/developerworks/java/library/j-jtp0618/index.html). As a standard workaround, it may implement Startable or define the event listener methods in an inner static class.
