A customer has requested the ability to have help tips for custom fields. I was thinking it would be nice to have options for both/or either alt text values and pop-up content values.

This would be useful. In our case we have a priority field and a custom severity field which both should be described the same way for consistency.

For example, a priority can be 'High, Medium, Low', while a severity might be 'Fatal, Medium, Cosmetic'.
An obscure crash, might have a 'Fatal' severity but a 'Low' priority. A crash on start-up will have a 'Fatal' severity' and 'High' priority etc. This makes it easier to track bugs and determine the order in which they should be fixed.

Because both Severity and Priority are closely releated, it would be nice if we could provide a brief description on how to use them the same way (i.e. both through the pop-up menu, or with a brief description underneath the field).

We have quite a few custom fields that would benefit from Help pop-ups. Users trip over some of our fields because we have done the most fundamental "No No" - changed meaning of fields on them. When can we expect this cool feature to be expanded to include custom fields?
