seems to be different from JRA-10074 – error looks different, this one didn't happen consistently, happened in ff.
was trying to attach the second screenshot, "view_mode.png", on this issue: CONF-11982 by clicking the link to upload another issue after already uploading one. got the error (see screenshot). tried again and it worked

Would you be able to tell us when this happened so we can trace it in our logs? If not, try to keep this in mind for the next time this happens.
Also, would it be possible that your session timed out between your last operation and when you clicked the link? Any more information that would shed more light onto this problem would be great.

not sure about session timeout, i clicked the link soon after the page loaded
