headlines (h1, h2,..) included inside code macro are rendered from toc and toc-zone macro

      when having wiki code inside a code macro like

      \h1. this
      blabla
      \h2. that
      blabla
      {code}

      and page uses {toc} or {toc-zone} the headlines are rendered inside the toc list too
