Audit username case issues

      There are a bunch of open issues (that I'll link here) to do with Confluence's schizophrenia about whether usernames are case-sensitive, case-insensitive or case-preserving.

      We should go through, once and for all, and find a

      • uniform approach that,
      • preserves bacwards compatibility
