Clickr Theme Missing some CAPTCHA fields

      This occurs using version 0.4 of the clickr theme plugin.

      With CAPTCHA turned on:

      • The first attempt to submit a comment always fails, whether or not the CAPTCHA phrase displayed is entered correctly.
      • No CAPTCHA fields are displayed when the "Empty Comment" error message is given.

      With CAPTCHA and threaded comments turned on:

      • When replying to a comment, no CAPTCHA fields are displayed.

      In cases where the CAPTCHA fields are not displayed, CAPTCHA still forbids the entries being made, and the fields are then shown on the error page.
