In the Rich Text 'Add Link' dialog, going to a different page of the search results removes the textboxes and buttons required to complete the form.

Steps to reproduce on confluence.atlassian.com:

1. Add a new page in the TEST space.
2. Go to the Rich Text editor.
3. Click on the 'Add Link' button.
4. Search for something.
5. Click on page #2 of the search results.

The textboxes and buttons on the form disappear, and clicking on any of the search results causes a Javascript error.
