Allow macro plugin definitions to specify a file that contains some HTML that should be inserted into the notation guide. The definition should state which page of the notation guide the HTML fragment should be inserted into (a macro can define a new page if it wants).
Disabled macros should not show up in the notation guide.
Do we need some way to order macros in the guide? (so that macro authors can ensure
{set-foo}and
{display-foo}are described next to each other)
